3/18/2024 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a foul mood evident by her facial expressions such as a frown on the face. QMHPA inquired about her morning and why she had the frown on her face as well as engaging in the use of profanity. QMHPA informed Ms. Broadnax that this session was going to focus on reducing her depressive symptoms and incorporating learned coping skills. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHPA inquired about Ms. Broadnax’s mental stability and engaged Ms. Broadnax in a discussion surrounding her medication compliance. QMHPA educated Ms. Broadnax on independent living skills and being able to function as a normal adult. QMHPA discussed with Ms. Broadnax about following daily living skills such as complying with medication regiments, bathing, changing clothes and hygiene regiments. QMHPA encouraged Ms. Broadnax in maintaining organization throughout the community and home. QMHPA discussed with Ms. Broadnax the importance of personal hygiene regiments and how it will improve her living skills when completed daily. QMHPA reviewed with Ms. Broadnax cleaning regiments such as washing clothes, separating clean clothes from dirty clothes and organizing her area in her room (getting hot to prevent rodents and other insects from entering the home). QMHPA engaged Ms. Broadnax in a skills building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to demonstrated proper cleaning techniques through role play Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax expressed to QMHPA that she struggles with her independent living skills because she doesn’t live on her own. Ms. Broadnax expressed to QMHPA that she really didn’t know a lot of independent living skills until she came connected with QMHPA. She discussed with QMHPA her mental health has a lot to do with it. Ms. Broadnax was quiet when asked about personal hygiene. Ms. Broadnax admitted does not practice proper hygiene regiments daily; however, she is a work in progress with it.
